Denver Post, NOTICE OF PUBLIC HEARING Notice is hereby given that the Colorado Health Facilities Authority (the "Issuer") will hold a public hearing at 10:30 am on July 6, 2015 at 3033 East First Avenue, Suite 301, Denver, Colorado 80206. The purpose of this public hearing is to consider the issuance by the Issuer of one or more series of its tax-exempt bonds and/or the borrowing and making of one or more tax-exempt loans by the Issuer, for the benefit of Catholic Health Initiatives, a Colorado nonprofit corporation ("CHI") and affiliates, in an aggregate principal amount not to exceed $150,000,000 (collectively, the "COHFA Debt"). The COHFA Debt will be used (i) to refinance, in whole or in part: (a) the Pulaski County, Arkansas Health Facilities Board (St. Vincent Infirmary) Variable Rate Demand Revenue Bonds (Catholic Health Initiatives) Series 2000B (the "AR Bonds"), (b) the County of Montgomery, Ohio's (the "Ohio Issuer") Variable Rate Revenue Bonds (Catholic Health Initiatives) Series 2004B (the "OH 04 Bonds") and/or (c) the Ohio Issuer's Variable Rate Revenue Bonds (Catholic Health Initiatives) Series 2008C-2 (the "OH 08 Bonds") and (ii) to pay certain expenses incurred in connection with the COHFA Debt and the refinancing of the above-referenced bonds. The Arkansas Bonds and Projects The proceeds of the AR Bonds were used to: (i) finance the payment or reimbursement of the costs of various capital expenditures (the "AR Projects") made by or on behalf of St. Vincent Infirmary Medical Center or their affiliates at facilities owned or operated by CHI, St. Vincent Infirmary Medical Center or their affiliates and located at the addresses listed below and (ii) paying certain costs of issuing and selling the AR Bonds as permitted by Title 14 Chapter 137 of the Arkansas Code. The AR Projects and the improvements financed or refinanced with the proceeds of the AR Bonds are owned or operated by CHI, St. Vincent Infirmary Medical Center or their affiliates and are located at the following addresses: In Little Rock, Arkansas - One, Two Five and 6101 St. Vincent Circle, 4202 South University, 10000 Rodney Parham Road, 2500 East 6th, 1100 North University, 500 South University, Suite 808, 1024 Scott Street, 6101 West Capitol, 820 Louisiana Street, 5918 Lee Avenue, 10101 Mabelvale Cutoff, 4208 North Rodney Parham and 1024 Scott Street; In North Little Rock, Arkansas - 5207 East Broadway, 2001 West Pershing Circle, 2003 and 2011 Fendley Drive, 3470 Landers Road, 1723 Schaer, 4800 East 19th Street and 4701 Fairway Avenue; In Sherwood, Arkansas - 2201 and 2215 Wildwood Avenue; In Jacksonville, Arkansas - 1110 West Main, #2 Crestview Plaza, and 813 Marshall Road. The Ohio Bonds and Projects The proceeds of the OH 04 Bonds were used to (a) pay the costs of certain capital improvements and equipment acquisitions at health facilities owned and/or operated by certain of the OH Affiliates listed below at certain of the OH Financed Locations listed below, (b) make a loan to Bethesda Hospital, Inc. ("Bethesda") to pay the costs of certain capital improvement and equipment acquisitions at health facilities owned and operated by Bethesda at certain of the OH Financed Locations listed below, (c) refund on a current basis the County of Hamilton, Ohio Hospital Bonds, Series 1994 (Sisters of Charity Health Care Systems, Inc.) (the "1994 Cincinnati Bonds") and the Ohio Issuer's Revenue Bonds, Series 1994 (Sisters of Charity Health Care Systems, Inc.) (the "1994 Dayton Bonds") and (d) pay costs of issuing the OH 04 Bonds and refunding the bonds refunded thereby. The proceeds of the 1994 Cincinnati Bonds were used to (i) pay or reimburse The Good Samaritan Hospital of Cincinnati, Ohio for the costs of acquiring, constructing, renovating, remodeling and equipping certain health care facilities owned by Good Samaritan Hospital of Cincinnati, Ohio at certain of the OH Financed Locations listed below and (ii) pay certain costs incurred in connection with the issuance of the 1994 Cincinnati Bonds. The proceeds of the 1994 Dayton Bonds were used to (i) pay or reimburse Good Samaritan Hospital (Dayton) for the costs of acquiring, constructing, renovating, remodeling and equipping certain health care facilities owned by Good Samaritan Hospital (Dayton) at certain of the OH Financed Locations listed below and (ii) pay certain costs incurred in connection with the issuance of the 1994 Dayton Bonds. The proceeds of the OH 08 Bonds were used to (a) pay the costs of certain capital improvements and equipment acquisitions at health facilities owned and/or operated by certain of the Ohio Affiliates listed below at certain of the locations listed below, (b) make a loan to Bethesda to pay the costs of certain capital improvement and equipment acquisitions at health facilities owned and operated by Bethesda at certain of the locations listed below and (c) pay bond insurance premiums related to the OH 08 Bonds.
