Times Union, STATE UNIVERSITY OF NEW YORK NOTICE TO BIDDERS The State University of New York Upstate Medical University will receive sealed bids for Project No. 1025 Titled "UH 2 South Locker Upgrade" until 2:30 PM Local Time on Tuesday July 14, 2015 at: Delivered to: Mr. Daniel Arnold Campus Purchasing SUNY Upstate Medical University Dr. Sarah Loguen Center, 2nd floor 650 South Salina Street, Room 2045 Syracuse, New York 13202 where they will be opened publicly and read. Proposals may be hand delivered or mailed to the above location and must be received at such time. Scope of work: is to include all necessary labor, material and equipment for: The removal and replacement of the existing lockers, along with the outdated finishes throughout the University Hospital 2 South corridor and elevator lobby with updated, modern equivalents. Much of the removal work will be performed by the abatement contractor and communication will be required to coordinate the completion of the new work. New gypsum board partitions and phenolic locker cabinets will be installed throughout the corridor. A new acoustic tile ceiling system, gypsum board soffits, LED light fixtures, rubber flooring, wall paint, wall protection, and handrails will all be installed in the project area. Two South Corridor work to be completed by October 20,2015, 77 calendar days. Reinstallation of the existing lockers in Weiskotten Hall and installation of new phenolic locker cabinets in the New Academic Building are also required. New Academic Building work to be completed by February 29, 2016. A mandatory site visit is scheduled for Monday, June 29, 2015 @ 10:00 AM with all contractors assembled at University Hospital, Second Floor Lobby, 750 East Adams Street, Syracuse, New York. All potential bidders are advised that they must sign a "Site Visit Form" to verify that they have complied with this requirement of the contract. Failure to fulfill this requirement will disqualify your bid proposal. All work on this Contract is to be completed within 209 calendar days starting ten (10) calendar days after the contract approval date of the State University of New York. Bidding and Contract Documents may be examined free of charge at the campus and at: SUNY Upstate, Office of Facility Design, 650 South Salina Street, 2nd Floor, Syracuse, NY 13202 Complete sets of Contract Documents for bidding may ONLY be obtained from: Syracuse Blueprint company, Inc., 825 East Genesee Street, Syracuse, NY 13210, PH. (315) 476-4084, Ph. (800) 962-1458, Fax (315) 476-7578. Contract Documents will be available after Monday June 22, 2015. Section 143 of the State Finance Law requires payment of a deposit to receive these documents. SUNY Upstate Medical will accept business checks, credit cards or cash for this deposit (no personal checks will be accepted). Accordingly, a non-refundable deposit check in the amount of $45.00, made payable to SUNY Upstate Medical University is required. Deposits less than $49.99 are non refundable. Effective 1/1/06, in accordance with State Finance Law Section 139-j, all questions pertaining to the solicitation shall be made formally to the following Asst. Bids must be submitted in triplicate in accordance with the instructions contained in the Information for Bidders. Security will be required for each bid in an amount not less than five (5) percent of the Total Bid. It is the policy of the State of New York and the State University of New York to encourage minority business enterprise participation in this project by contractors, subcontractors and suppliers, and all bidders are expected to cooperate in implementing this policy. The selected contractor must comply with M/WBE goals. State University of New York goals are: 15% MBE and 15% WBE participation of the total contract value. State University of New York also has a goal for the Work Force participation: EEO 10% WBE and EEO 10% MBE of total work force on the project.
