Times Union, REQUEST FOR PROPOSALS FOR HEALTH CARE SERVICE PROVIDER The Town of Prattsville is applying for Community Development Block Grant-Disaster Recovery ("CDBG-DR") funds from the New York State Governor's Office of Storm Recovery ("GOSR"), NY Rising Community Reconstruction ("NYRCR") Program for the Prattsville Health Care Facility Project. The Town of Prattsville is interested in procuring the services of a Health Care Provider to enter into a negotiated 5-year long-term lease, with 5-year renewal(s) thereafter, to operate, manage and finance the equipment, staffing, and supplies to provide the required services over the term of lease necessary to operate a regional health care facility, insuring sustainability and efficiency in providing competitive health care at the highest levels for the region of the health care facility, if successful in being funded under NYRCR Program. The procedures for the selection of this Health Care Provider will be in accordance with the procurement requirements of the CDBG-DR funds from the GOSR, NYRCR Program. All responses received will be evaluated in accordance with the selection criteria and corresponding point system, which is identified in the Request for Proposals package. That package also identifies the scope of services to be performed by the selected firm. The Town of Prattsville will award the contract to the respondent obtaining the highest score in the evaluation process.
